Case Manager / Care Coordinator

Coordinate services, help clients find housing, and guide them through Personal Care Management and transitional assistance.

What you'll do

• Manage a caseload of seniors and people with disabilities
• Coordinate home care, transportation, and community services
• Help clients search and apply for subsidized and accessible housing
• Support MassHealth PCA (PCM) intake, orientation, and paperwork
• Document services and outcomes in our MyCase platform

What you need

• Bachelor's degree in human services or a related field preferred
• Case management or community services experience
• Strong organization and communication skills
• Valid driver's license
