Most private home care agencies in Massachusetts set a minimum: usually 3 or 4 hours per visit, sometimes a weekly floor such as 12 or 20 hours, and often a signed service agreement. If your parent needs one hour in the morning, a 4-hour minimum means paying for three hours nobody needed.

What minimums are common?

  • Per visit: 3 or 4 hours is the norm. Agencies say it is needed to attract caregivers, since short shifts are hard to staff.
  • Per week: some agencies require 12, 16 or 20 hours a week before they will start service.
  • Contracts: a service agreement with a notice period to cancel, and sometimes a deposit.
  • Assessment fee: some charge for the first home visit before care starts.
  • No published price: many agencies only quote after an in-home consultation.

What does a 4-hour minimum actually cost?

Take a parent who needs help getting up, washed and dressed, which is about 90 minutes. With a 4-hour minimum, five mornings a week is 20 billed hours. Paying only for the 90 minutes is 7.5 hours. At typical Greater Boston rates, the difference is several hundred dollars a week.

When is a longer visit the right call anyway?

If your parent has dementia, cannot be left alone, or needs a meal, a bath and medication reminders spread across the day, a longer block makes sense. The point is that the care plan should set the hours, not the agency's billing rule.

What should you ask before you sign?

  • What is the minimum per visit and per week?
  • Is there a contract, a deposit or a cancellation notice period?
  • Is the hourly rate the same on weekends and holidays?
  • Is there a fee for the assessment?
  • How will I know the aide arrived and how long they stayed?
  • What happens if the aide calls out?

How does Care Remedy handle this?

Home health aide care is $45 an hour, booked from 1 to 8 hours a day, with no minimum contract and no assessment fee. Overnight is $540 per 12-hour night and around-the-clock care is $1,080 a day. Every visit is logged in MyCase, our coordination platform, so families can see when the aide arrived and left.

Our aides are trained, background checked and insured. They are not nurses and do not administer medication; nursing is a separate service.

Are there free options to try first?

Yes. Your local Aging Services Access Point (ASAP) can assess your parent for state home care programs, and the town Council on Aging often runs rides and check-in calls. If your parent is a veteran, ask about VA Aid and Attendance.

The valet entrance and the pickup point are on two different streets, valet stops at 4, and one of the two lots is not wheelchair accessible. What that means after eye surgery.
